At Canadian Institute of Bariatric Options, we respect our client's rights to personal privacy. It has always been our priority to safeguard any information provided by our clients and we are committed to full compliance with Privacy Laws that relate to our business.
Our privacy policy is based on the ten principles that are set out in the Canadian Federal Privacy Law and are the basis of the Privacy Laws in the provinces in which we do business.
This statement is meant to describe what type of identifiable information about our clients ("Personal information") we collect and use, why we do so and whether and to whom we disclose it. This statement is also meant to describe how you can contact our Privacy Compliance Officer with inquiries or complaints, or to exercise your rights under the Privacy Laws, including your right to access and contact your personal information.
COLLECTION AND USE OF PERSONAL INFORMATION
Canadian Institute of Bariatric Options collects and uses personal information primarily for the purpose of administering clients' records and conducting internal reviews, investigations and complaint resolution processes. Personal information that Canadian Institute of Bariatric Options may collect includes: The individual's name and the address and other contact information, such as telephone numbers and e-mail addresses. Designation or a title and sometimes signatures of individuals on contracts. Unless a client specifically authorizes us to release personal information, or release is required or permitted by law, Canadian Institute of Bariatric Options will not sell, lease, or trade personal information to other third parties.
Canadian Institute of Bariatric Options will only ask for information that we need and will only ask for it when we need it. When we ask for the information, we will explain why we need it and what we are going to do with it.
We will seek client's prior consent to the collection, use, and disclosure of the information as required by applicable privacy legislation. Subject to legal and contractual requirements, a client can refuse to consent to our collection, use, or disclosure of information. Clients may also withdraw their consent to any further collection, use, or disclosure of information about themselves at any time by giving us reasonable notice. If our intended disclosures or uses of information change, we will notify them accordingly.
Client's personal information will not be used for any other purpose without his or her consent.
DISCLOSURE
Personal information is disclosed on a need-to-know basis to Canadian Institute of Bariatric Options and to selected third parties who are acting on our behalf as our representatives, suppliers, or service providers. Such third party service providers are provided only with such information as is necessary in the circumstances (i.e. individual client information may be disclosed to our Insurance providers). Information provided to third parties may be used only for the purpose stipulated and is subject to strict terms of confidentiality.
